What to Do in the Event of Death

Call The Daleiden Mortuary at (630) 631-5500

Inform The Daleiden Mortuary of the following information:
Who you are
Your telephone number
Who has passed away and at what time
Where the deceased is located

Make an appointment with The Daleiden Mortuary

Gather the following information for the death certificate:
Full name and middle initial of the deceased
Date and place of birth
Social Security number
Marital status
Father's name
Mother's maiden name
Education
Military record
Select clothing and personal items for the deceased such as several recent photographs, a full set of clothing including undergarments, eyeglasses, dentures, jewelry, and religious symbols such as a cross, rosary, etc.

Gather obituary information. The Daleiden Mortuary normally writes the article and submits it to the newspaper(s).

Select pallbearers

Eventually, other matters must be attended to such as:
Locate the will and notify the lawyer and executor
Check and notify:
All life and casualty insurance companies
Death benefits and/or credit union
Trade union
Fraternal organizations
Social Security

The Daleiden Mortuary will prepare Social Security Form SSA 721. If checks are automatically deposited, notify the bank of the death. Call the Social Security Administration at (800) 772-1213 for possible additional benefits and to make sure that the number is retired

If the deceased was living alone, notify:
Landlord
Utility companies
Post office-give a forwarding address
